Event Readiness Check

If your team has “Not Event Ready” in the “HQ Event Ready” column, please review the following items for completion:

  • Team has both a Lead Coach/Mentor 1 and 2 listed on the Team Dashboard.
  • Both Lead Coach/Mentor 1 and 2 have electronically signed their FIRST HQ Consent/Release form (indicated on the individual’s FIRST Account).
  • Both Lead Coach/Mentor 1 and 2 have completed the FIRST Youth Protection background screening (indicated on the individual’s FIRST Account).

FIRST in Michigan Consent/Release Forms

FIRST in Michigan Consent/Release Form:

  • The FIRST in Michigan Consent/Release form is a separate and additional form from the FIRST HQ Consent/Release form.
  • All team participants (Lead Coach/Mentor 1 and 2 and team members) must have an electronically signed FIRST in Michigan Consent/Release Form on file before their event date. This is in addition to the FIRST HQ Consent/Release Form.

Checking on FiM Consent/Release Form status for your team

To check on the status of your team’s FIRST in Michigan Consent/Release forms:

    • Enter your team number in the “Search” box.
    • Click on the “+” sign to expand the information for your team.
      • The listed entries indicate the individual for whom a FIRST in Michigan Consent/Release form has been received.
        • Display name = Program+First Name+First Initial of Last Name.
        • The table shows a maximum of 17 display names. If your team participant count exceeds this number, email your program partner to obtain a full list of team display names.
    • If a team participant believes they have submitted a FiM Consent/Release Form and their display name doesn’t show up under your team number, it could be due to a transposition of numbers or inclusion of text in the team number field when the participant filled out the form.

Use of the Display Name field on Team Participant Roster

For teams competing in ULTIMATE GOAL remote events:

  • The “display name” field will be used by the team member to gain entry into the team’s virtual judging session on Zoom.
    • Reminder that FTC team size is capped at 15 team members.
  • Each team may have one mentor participate as a silent observer to the team’s judging interviews. That mentor must also have a signed FIRST in Michigan Consent/Release form on file.

Team Participant Roster for ULTIMATE GOAL remote events:

  • Team Participant Roster Template: Google Sheet; download to customize.
  • Teams are required fill out and send their event’s Event Coordinator a “Team Participant Roster” by Wednesday, 8pm of their remote event week.
  • The team roster with corresponding display names is the “ticket” for team participants to gain entry into the team’s virtual judging session. This check-and-balance is in place to protect our youth team members without sharing personal identification information.
  • Each participant’s “display name” listed on the roster must also appear in the team’s entry on the table below. This provides the cross-reference that the team participant has a signed FIRST in Michigan Consent/Release form on file.
  • Team participants that do not have a FIRST in Michigan Consent/Release form on file and thus do not appear in the table below, will not be admitted into the team’s virtual judging sessions.
    • Teams are limited to one mentor participant, as a silent observer, to the team’s judging interviews regardless of how many have FIRST in Michigan Consent/Release forms on file.
