Functionality has been added to the FTC Live system to support FTC judging. FIRST FTC HQ has now made FTC Live the &#8220;Plan A&#8221; approach for FTC remote event administration. font-size: 1em;\"><span style=\"color: #ff6600;\">FIRST IN MICHIGAN CONSENT\/RELEASE FORM<\/span><\/strong><\/h5>\n<p>The FIRST in Michigan FTC Consent\/Release form is now available to be filled out electronically via Smart Waiver: <strong><a href=\"https:\/\/waiver.smartwaiver.com\/v\/firstinmichigan\/\" target=\"_blank\" rel=\"noopener noreferrer\"><i>FIRST<\/i>\u00a0in Michigan Consent and Release Form<\/a><\/strong> (link).<\/p>\n<p>Every team member, coach\/mentor participating in an official engagement option in the 2020-21 season must fill out this form.<\/p>\n<h5><strong style=\"color: #000000; font-size: 1em;\"><span style=\"color: #ff6600;\">2020-21 EVENT PARTICIPANT REGISTRATION AND CONSENT\/RELEASE FORMS\u00a0<\/span><\/strong><\/h5>\n<p>Below is a recap of the event participant registration and Consent\/Release form requirements.<\/p>\n<p>There are two different Consent\/Release forms required:<\/p>\n<ol>\n<li><strong>FIRST HQ Consent\/Release form<\/strong> is filled out via the participant&#8217;s FIRST Account accessed through https:\/\/www.firstinspires.org\/ (top right &#8220;Register \/ Login&#8221;.<\/li>\n<li><strong>FIRST in Michigan Consent\/Release form<\/strong> which can now be filled out electronically through Smart Waiver.<\/li>\n<\/ol>\n<p><strong>Youth team members:<\/strong><\/p>\n<ul>\n<li aria-level=\"1\">All youth team members must register with FIRST HQ through the\u00a0<a href=\"https:\/\/www.firstinspires.org\/resource-library\/youth-registration-system\" target=\"_blank\" rel=\"noopener noreferrer\">Youth Registration System<\/a>.\n<ul>\n<li aria-level=\"2\">This process includes the ability for the parent\/guardian to electronically sign the FIRST HQ Consent\/Release form.<\/li>\n<\/ul>\n<\/li>\n<li aria-level=\"1\">Must\u00a0<b>ALSO<\/b>\u00a0complete the\u00a0<a href=\"https:\/\/waiver.smartwaiver.com\/v\/firstinmichigan\/\" target=\"_blank\" rel=\"noopener noreferrer\"><i>FIRST<\/i>\u00a0in Michigan Consent and Release Form<\/a>\u00a0electronically.<\/li>\n<\/ul>\n<p><strong>Coaches and mentors:<\/strong><\/p>\n<ul>\n<li aria-level=\"1\">Each team is required to have a Lead Coach\/Mentor 1 and Lead Coach\/Mentor 2 listed on the Team Dashboard.\n<ul>\n<li aria-level=\"2\">Coaches and mentors register with FIRST HQ through the\u00a0<a href=\"http:\/\/firstinspires.org\/\" target=\"_blank\" rel=\"noopener noreferrer\">FIRST Dashboard<\/a>.<\/li>\n<li aria-level=\"2\">This process includes the ability to electronically sign the FIRST HQ Consent\/Release form.<\/li>\n<li aria-level=\"2\">Both Lead Coach\/Mentor 1 and 2 must complete the FIRST Youth Protection Policy (YPP)\u00a0<a href=\"https:\/\/www.firstinspires.org\/sites\/default\/files\/uploads\/about\/us-screening-steps-2019.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">background screening process<\/a>.<\/li>\n<\/ul>\n<\/li>\n<li aria-level=\"1\">Must<b>\u00a0ALSO<\/b>\u00a0complete the\u00a0<a href=\"https:\/\/waiver.smartwaiver.com\/v\/firstinmichigan\/\" target=\"_blank\" rel=\"noopener noreferrer\"><i>FIRST<\/i>\u00a0in Michigan Consent and Release Form<\/a>\u00a0electronically<\/li>\n<\/ul>\n<p><b>Note:<\/b>\u00a0We recognize that not all teams have the necessary internet access to complete the online registration process.<\/p>\n<ul>\n<li aria-level=\"1\"><b>On an exception-only basis<\/b>, team Lead Coach\/Mentor 1 or 2 may request a paper copy form of both the FIRST and FIRST in Michigan consent and release forms from their FIRST in Michigan program director.<\/li>\n<li aria-level=\"1\">Per FIRST HQ, once signed, the only approved methods for the collection of these forms is to either drop them off in person or mail them first-class to your FIRST in Michigan program director.
