\nRequired at each event where a team member\/coach\/mentor is participating. Two versions for each person must be turned in at event check-in.<\/p>\n

FIRST HQ Consent and Release Form<\/a><\/strong>
\nElectronically signed through the individual’s FIRST TIMS Account. Team must be using the Team Dashboard produced roster as it indicates that the electronically signed form is on file with FIRST HQ.
\nOR<\/strong> a signed paper version.<\/p>\n

FIRST in Michigan Consent and Release Form<\/a><\/strong><\/p>\n
